Referrals can be made by professionals working with the families, with their permission and from family members themselves.
If you would like to talk with someone about the service in more detail or wish to make a referral on behalf of a family then please call our team on 01452 397692. Alternatively please email: familyfocus@nelsontrust.com
What Next :
On receipt of the referral the relevant information will be passed to the team and allocated to a member of staff who will contact the referrer to arrange a family meeting. Following an assessment a structured care plan will be generated and agreed with all those concerned, in compliment with any other professional involvement.
